WAIVER + INTAKE CONTRACT
Your pet is very important to us. The team at Applehead would like to assure you that every effort will be made to make your pets grooming experience as safe and enjoyable as possible. Safety comes first for everyone, people as well as the pets, during the grooming process.
Health + Medical
Occasionally, grooming can expose a hidden medical problem or aggravate an existing one such as but not limited to hot spots, skin allergies, scabs, wounds, heart, trachea and lung complications. All medical expenses for veterinary care for such previous existing problems will be covered by the pets owner upon signing this agreement.
Flea + Tick Policy
Applehead strives to be a clean and flea/tick-free salon. If your pet has fleas/ticks, they will be given a flea/tick bath at your expense. If you do not want your pet to receive one, you will need to pick up immediately to prevent infestation of other pets and reschedule your appointment after the flea problem has been addressed.
No-show + Cancellation Policy
We ask that any cancellations or appointment changes be made at least 24 hours in advance. We are a small business, whose employees are commission based. If there is a no-show or a cancellation is made after 24 hours of the appointment, there will be a $40 fee charged to the card on file (certain exceptions can be made) Repeated no-shows or cancellations will require advance payment of next appointment by credit card.
Pick-Up Policy
Applehead is a very unique and one of a kind salon that is 100% no-crate/kenneling. To ensure proper space for every pet, we require immediate pick-up. Applehead will give you a 15-30 minute heads up of when your pet is expected to be finished. If the pet is failed to be picked-up promptly, there may be a late-pick up fee added to your invoice for every 15 mins after your notified pick-up time to compensate a team members time of individually occupying your pet.
Matting
Pets with severely matted coats require extra attention. Mats in a pet’s coat grow tight and can ultimately damage and tear the pet’s skin, which provides a breeding ground for parasite infestations, and/or severe-mild skin irritations. Applehead will not cause serious or undue stress to your pet by dematting by brushing. Mats can be very difficult to remove, and may require the pet to be shaved. Removing a heavily matted coat could possibly cause nicks, cuts, or abrasions due to skin growth trapped in the mats. Heavy matting can also trap moisture and urine near the pet’s skin allowing mold, fungus or bacteria to grow, producing skin irritations that existed prior to the grooming process.
After-effects of mat removal procedures can include itchiness, skin redness, self-inflicted irritations or abrasions and failure of the hair to regrow. In some rare/extreme matting cases, pets may also exhibit brief behavioral changes. Prevention is the best defense by scheduling regular grooming appointments. If your pet needs to be shaved to remove matting, by signing below you agree to this procedure, at any risk. There could be an additional charge for this process; it is time-consuming and we want to ensure the comfortability and safety of your pet and team members. Additional charges fluctuate depending on the severity of the matting and size of the pet. By signing below, you give Applehead full permission to style your pet as they deem professionally safe and best for your pet.
Accidents
Although accidents are rare, there is a risk when dealing with pets. Grooming equipment is sharp, and although we use extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of the nails, etc. In most cases this can happen when a pet is wiggling or moving around. Your pet’s safety and comfort is our number one priority, if the pet or team member feels that safety may be compromised we will stop the grooming process. However, in the event an accident does occur, you will be notified of the accident. If Applehead feels it is serious and the owner is not on-site, a team member will seek immediate veterinary care for your animal at a vet of our choice.
